Transformational Collaborative Outcomes Management (TCOM) Orientation and CANS Overview
The Child and Adolescent Needs and Strengths-Commercially Sexually Exploited (CANS-The Santa Clara County Child and Adolescent Needs and Strengths 2.0 (Ages 5+) is a collaboratively completed measure of youth and family strengths and needs developed specifically for youth who have experienced sexual exploitation. This evidence- based, standardized assessment was developed to support decision-making, including level of care and intervention planning, to facilitate quality improvement initiatives, and to allow for the monitoring of clinical and functional outcomes for sexually exploited youth. As a communication tool, it facilitates the linkage between the assessment process and the design of individualized service plans. This workshop provides an overview of the Santa Clara County CANS 2.0 5+ and Transformational Collaborative Outcomes Management (TCOM). TCOM’s overall framework, key concepts and how its multilevel approach directly benefits children and families will be discussed. The principles and best practices in using the CANS and the Commercially Sexually Exploited Module as a TCOM assessment strategy, a communication framework, and a tool to monitor outcomes and inform care plans for sexually exploited youth will also be addressed.
Participants will:
• Articulate the philosophy Transformational Collaborative Outcomes Management (TCOM) and how it guides
the development of a shared vision for children, families, providers and systems.
• Demonstrate understanding of the role of structured assessments in the mental health system as
demonstrated by the ability to identify the linkage between the assessment and the treatment planning
process.
• Describe the purpose of the CANS.
• Demonstrate understanding of how the Child and Adolescent Needs and Strengths is be used to support
system of care philosophy through identifying a shared vision for the treatment team.
Target Audience: Santa Clara County Behavioral Health Services Staff and contracted agencies.
